Worked it out in the end; the spigot appears to be some form of locking mechanism stopping the bulb holder from twisting.
After struggling to change the passenger bulb and in a bout of frustration with the driver side I ended up bending the spigot and the end came out! Once this was out the bulb holder can easily be twisted and removed.
Whoever at Ford designed this stupid arrangement and the fact that you have to dismantle the wheel arch liner to change a bulb should be made to do the job a hundred times!
